Community Health Systems, Inc. is constantly involved in Community Events and Outreach activities. A proactive approach is taken to inform and educate the public
of important health information regarding anything from family planning to diabetes.
Informational brochures and counseling is given by our staff and many times we offer
on-site eye exams and blood pressure readings.

iTeen, a teen clinic, is available for teens 12-19 years old every Thursday from 3-5pm. This is a confidential venue for teens to get health services and accurate health information. We have music, games, take-home snacks, and giveaways for teens.

On Friday, August 5, 2011 our clinic will be hosting a health fair to celebrate a kick-off to Health Center Week and to introduce our Providers to the community. Expect No Cost entertainment, gift raffles, health screenings, & booths with goodies from local vendors. Health Fair from 11am-3pm. Stay for light refreshments from 4-5pm & Meet our Providers.

Fallbrook Women's Health Center
In April the Fallbrook Family Health Center held its
first diabetic management class entitled “Etiology of
Diabetes.” The class is part of a six (6) series program
designed to help patients manage their chronic
disease. “Etiology of Diabetes” was presented jointly
in both English and Spanish by Claudia Chagolla, LVN
and Amparo Magana. The patients showed interest
and understanding of their condition by asking
several questions and sharing personal experiences.
The course was offered to a small group of fifteen (15)
people. Our goal in administering these classes is to
educate and monitor the same group of patients over
the six (6) month period to ensure progress in diabetic

Fallbrook, California
Every year in April the community of Fallbrook organizes its
annual Avocado Festival. It is the single most anticipated
event that the community of Fallbrook experiences. The
estimated attendance at these festivals is anywhere between
75,000 and 80,000 people. Fallbrook Family Health Center
has participated in the festival for four (4) consecutive years
as a non-profit organization by setting up a booth where
healthcare information is disseminated to the crowd. This
year the incentive items were provided by Molina Healthcare
and Anthem Blue Cross. It is estimated that we encountered a
total of 400 people, including 100 children, at this years
Avocado Festival.

Fallbrook Women's Health Center
In observance of National STD Awareness Month, the Women’s
Clinic organized an “STD Prevention and Awareness” event.
On April 18, 2008 Women’s Clinic accommodated an audience
of twenty-six (26) people: seventeen (17) female and nine (9)
male. In addition, three (3) key community representatives
attended the event: Sadalya Wilson and Elena Monreal from
SANDAPP and Tracee Matten from our local Women’s Resource
Center. The event consisted of a lecture, trivia game, clinic
tour, and a Questions & Answers session. The three (3) most
common STDs (Chlamydia, Herpes Simplex Virus (HSV), and
Genital Warts (HPV)) were thoroughly discussed. We thank

April 5, 2008
Fallbrook Family Health Center participated in the 7th annual
“Celebrate Your Health” Fair on April 5, 2008. A staff of
nine (9) was made available to provide no-cost dental, vision,
and blood pressure screenings. In addition, an exhibitor’s
booth was occupied by the health center where educational
material and incentive items were distributed. The estimated
attendance at the fair was 555. As a whole, we were able to
provide twenty (20) dental screenings and twenty-one (21)
Oral Health Initiative dental screenings to children ages zero
(0) to five (5), forty-six (46) vision screenings and fifty-nine
(59) blood pressure readings. Also, forty-five (45) people
approached the exhibitor booth where information on the
Healthy Families/Medical Programs and the services
Fallbrook Family Health Center offers was provided.

Fallbrook Women's Health Center
In March, Fallbrook Family Health Center provided no-cost
dental and vision screenings to a preschool class at Vallecitos
School located in Rainbow. The area of Rainbow is highly
populated with undocumented, migrant workers that have less
means of accessing healthcare. The event consisted of a brief
fifteen (15) minute presentation by Eva Viret, RDA; simultaneous
dental and vision screenings; a brushing session with each
child; and concluded with an educational video on oral health.
The screenings were provided to twenty-four (24) students all
between the ages of three (3) and five (5). There were at least
nineteen (19) appointments as a result of this event, most of
which were kept.

March 11, 2008
Fallbrook, California
On March 11, 2008, Sharna Williams, MS, CNM,
OB/GYN-NP, presented an informative talk at Potter
Junior High School entitled “Diet, Nutrition, Exercise
and General Health.” The lecture encompassed the
following topics: diet, nutrition, exercise, smoking
prevention, pap smears, annual breast exams, the
Human Papilloma Virus vaccine “Gardasil” and
diabetes. Sharna spoke to a group of twenty (20) girls
ages thirteen (13) and fourteen (14). Through a
questionnaire given to the attendees we found that
ninety-five percent (95%) are interested in learning
more about birth control. A follow up event has been
scheduled at the Women’s Clinic entitled “Methods of
Birth Control.
