What We Do

Community Health Systems, Inc. (CHSI) is a Federally Qualified Health Center (FQHC) and Non-Profit 501 (C)(3), operating six community health centers in the tri-county area of: San Bernardino, Riverside, and San Diego. 

CHSI focuses on providing high quality and compassionate medical, dental, women’s health, behavioral health, chiropractic care, and vision care services to those who are under-served, uninsured, or under-insured.

Federally Qualified Health Center

Community Health Systems, Inc. is a non-profit 501 (c) (3) 330 HRSA Grantee with Federally Qualified Health Center (FQHC) status. Established from the roots of Bloomington Community Health Center in Bloomington, CHSI has grown with community health centers in the counties of Riverside, San Bernardino, and San Diego. These centers have been developed in accordance with standards established for safety net providers by the U.S. Department of Health and Human Services (HHS), the Health Resources Services Administration (HRSA), the Public Health Service (PHS), and the Bureau of Primary Health Care (BPHC). The mission of CHSI is not to compete with other clinics that provide healthcare services, but to serve the indigent population and meet their healthcare needs. CHSI is a Health Center Program grantee under 42 U.S.C. 254b, a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n), and therefore receives Malpractice insurance coverage under the Federal Torts Claim Act (FTCA).

Community Health Systems Inc. is proud to be recognized for being a Patient-Centered Medical Home (PCMH), certified at level III for all health centers.


What does it mean to be PCMH certified?

Our PCMH certification means that our health centers have continued to put our patients at the center of health services by connecting them with our expert providers and care teams. Using coordinated care and ongoing quality improvement, our model has enhanced the lives of our patients. Our resilient care teams work to recognize and support patients and their unique needs by approaching their care through a lens of coordinated care.
