What if I don't have insurance?
We offer a Sliding-Fee-Scale (SFS) to individuals who cannot afford or do not qualify for healthcare coverage. Our medical, dental, women’s health, behavioral health, chiropractic and vision care services are all available through the SFS program, which is based on Income and Family Size.
What insurance does CHSI accept?
Community Health Systems, Inc (CHSI) accepts Medi-Cal, Medicare, and Private Insurance. Please contact your local Community Health Center to verify your health plan is accepted.
What services are offered at CHSI?
Adult Medical, Pediatric Services, Women’s Health, Prenatal Care, Senior Medical, Dental Care, Behavioral Health, Chiropractic Care, and Vision Care are all available throughout San Bernardino, Riverside, and San Diego Counties. Services will vary by location.
How can I become a patient?
Its easy! Call your local Community Health Center, and make an appointment as a new patient. We can assist you in changing your provider through your insurance. If you do not have insurance, and are using our Sliding-Fee-Scale program, you can make an appointment as soon as today!

What should I expect at my first appointment
The first step in enrolling a new patient at CHSI will vary, depending if you have insurance or not. Some patients may need to change their assigned doctor with their insurance, or some patients may need to enroll into a health program such as Medi-Cal, which could create a longer enrollment process with our Patient Services Representatives.
All new patients will be asked to complete a health questionnaire prior to seeing their provider, in order to have the best understanding of their health needs.

Are walk-ins accepted?
Yes! All our Community Health Centers accept walk-in patients. We always suggest to try and make an appointment, as extended wait times may exist for walk-in patients.

What transportation resources are available?
Some health insurance plans offer transportation services to their members (contact member services, or the number on your insurance card).

Where can I enroll into Medi-Cal Plans?
All CHSI Community Health Center can enroll people into Medi-Cal, apply for Presumptive Eligibility, or apply for temporary Medi-Cal for kids (CHDP).
Where can I enroll into Covered CA?
All CHSI Community Health Center can enroll people into health insurance coverage through the Covered CA marketplace. If it is not during the open enrollment period, you can only enroll into private health coverage due to a qualifying life event.
Federally Qualified Health Centers are community-based health care providers that receive funds from the HRSA Health Center Program to provide primary care services in under-served areas. Federally Qualified Health Centers may be known as Community Health Centers, Migrant Heath Center, Health Care for the Homeless, and Health Centers for Residents of Public Housing.

Yes, translators are available to all patients, free of cost. During your enrollment as a new patient, our patient service representatives will determine the type of translator needed.
