Enhanced Care Management

Extra Care, When You Need It Most!

If you have Medi-Cal, you may be eligible for Enhanced Care Management (ECM)

ECM offers extra services to help you get the care you need to stay healthy.

If you have complex needs that make it difficult to improve your health, we are here to help!

These services have no cost, and you can join or stop at any time.

What Does ECM Include?

ECM does not change any of your benefits. You keep your providers and get help to: 

  • Find doctors and get appointments for all health needs.

  • Improve your health by working with you and your doctor on your care plan.

  • Arrange transportation to doctors visits.

  • Get follow-up services after you leave the hospital.

  • Manage your medicines.

  • Connect to local resources such as food or other social services.

Who is On Your Care Team?

  • Nurse Care Manager: Supports Members with complex medical conditions and completes medication reconciliations. Provides Members and family education about chronic medical and behavioral health conditions for health promotion and improve health literacy.

  • Behavioral Health Care Manager: Supports Members with behavioral health conditions, such as serious mental illness or substance use disorders, through brief interventions and behavioral activation strategies.

  • Care Coordinator: Provides care coordination and connection to services including appointment and transportation scheduling and referral management.

  • Community Health Worker: A field‐based member that promotes collaboration between Member and their medical/behavioral team, increases sense of control over their health and to provide community services and supports through resources and referrals.
