Holiday Notice: All CHSI locations will close at 3pm on December 31st, and will remain closed on January 1st, 2025, in observance of New Year's Day. We will reopen on Thursday, January 2nd, 2025, with normal business hours.

Holiday Notice: All CHSI locations will be closed on Monday, February 17th, in observance of President's Day. We will reopen on Tuesday, February 18th, with normal business hours.

NOTICE: All CHSI locations will have modified hours and close early on March 15th, 2024. Please contact your nearest CHSI Health Center for their specific closing times.

First Visit

  • Photo ID 

  • Insurance card(s)

  • Proof of Income

Every Visit

  • Insurance card(s)

  • List of current medications: prescribed and over-the-counter

  • Discharge information from Urgent Care or Emergency Room visit

Appointments & Walk-ins

Walk-ins are welcome!

To provide you with the best possible service please schedule your appointment in advance.

If you did not make an appointment, you may still be able to see a provider as a walk-in (extended wait times may exist). Contact your local health center for more information.
